Symptoms typically persist for a longer time frame in patients taking drugs which have a long elimination half-life, active metabolites, or a protracted duration of action. Cases have reported muscle pain and weakness persisting for months, although antidepressant discontinuation may contribute to ongoing features. Upon initiation of therapy and the discontinuation of serotonergic drugs, most cases of serotonin syndrome resolve within 24 hours, although delirium may persist for a number of days. Following appropriate medical management, serotonin syndrome is generally associated with a favorable prognosis.
Differential diagnosis may become difficult in patients recently exposed to both serotonergic drugs and neuroleptic drugs.
Severe serotonin syndrome can be fatal if not treated. Milder forms of serotonin syndrome may go away within a day of stopping the medications causing symptoms and, sometimes, taking drugs that block serotonin. The first time this happened, we had no clue what was going on. My husband would fall.

There are, however, many other possible symptoms, including seizures and severe joint pain (Physical Therapy, June 2008).
My PM doc put me on tramadol ER and reg for breakthrough pain and started low on cymbalta. After one year I was taking 200mg Tram ER + 200mg Tram throughout the day and 60mg cymbalta. I got slammed with serotonin syndrome and then the absolute nighmare of getting off cymbalta. The Tram was.
