Serotonin syndrome (also known as “serotonin toxicity”).

It has been a popular choice for treating pain because it is safer than other opioids, due to its lower potential for dependence and lower propensity to cause respiratory suppression. Most cases of serotonin syndrome reported in patients taking tramadol occur when tramadol is combined with other serotonergic agents, especially those that inhibit CYP2D6, including antidepressants. Combining tramadol with certain serotonergic medications is contraindicated because of the risk of serotonin syndrome. As a racemic mixture of (+)-tramadol and (−)-tramadol, the medication's (+) and (−) enantiomers can inhibit serotonin reuptake and (−)-tramadol can promote serotonin release. 5-Hydroxytryptamine, or serotonin. As with almost all medications with serotonergic activity, tramadol can cause serotonin syndrome. Tramadol is a phenylpiperidine medication structurally related to codeine and morphine.
